Job Description:

St. Gabriel's Hospice and Palliative Care is seeking a compassionate and dedicated PRN Social Worker to join our team in the north part of Houston, Texas. As a Social Worker, you will provide essential emotional and social support to patients and their families during their most challenging times. Your expertise will help our patients navigate the complexities of end-of-life care with dignity and respect.

Key Responsibilities:

• Conduct comprehensive psychosocial assessments for hospice patients.

• Develop and implement individualized care plans to address the emotional, social, and practical needs of patients and their families.

• Provide counseling and support to patients and families facing terminal illness, grief, and loss.

• Collaborate with the interdisciplinary team to ensure holistic and patient-centered care.

• Facilitate access to community resources and services as needed.

• Maintain accurate and timely documentation of all patient interactions and interventions.

• Participate in team meetings and case conferences to discuss patient care and progress.

Qualifications:

• Master's degree in Social Work (MSW) from an accredited institution, preferred.

• Current Social Work license in the state of Texas.

• Minimum of 2 years of experience in hospice, palliative care, or a related field, preferred.

• Strong understanding of the psychosocial aspects of end-of-life care.

• Excellent communication and interpersonal skills.

• Ability to work independently and as part of an interdisciplinary team.

• Compassionate and empathetic approach to patient care.

• Flexibility to work on an as-needed basis (PRN).
